Made while sourcing, by people who are not thinking about a reporting obligation at all. Done that way, the report at the end is a query. Not done, it is a reconstruction from purchase orders and memory, in the weeks when there is least room for it.
The gaps are the hardest part and the most important. An unidentified tier is the single most useful thing on the page, and it is exactly what a spreadsheet renders as a blank cell that nobody's eye stops on.
